Again, apart from it being the writer's lazy attempt to bring Ayra, Zaid, MJ (and definitely Asif too!) under the same roof, without being biased to any character, the polygamy situation is the fault of all the three of them - Zaid, MJ and Ayra, all three of them are responsible for the soup that they are currently in (and we all are currently simmering within!).
I will start with MJ - she was afraid of the social stigma of being a second time divorcee and being proved Mohib right on the comment that she'll be divorced 100 times (I also think there is another reason, but I'll keep it aside for the time being!), so she was ready to preserve her marriage any which way possible - even as to going into lengths of accepting a second wife in her husband's life. So, yes, the 'two-wives' situation definitely has her hand!
Then comes Ayra - she refused to marry Zaid when he said he'll divorce Mubashira, and was ready to move out of town. Then, she put forth the condition of family acceptance. Now, I first thought it was her way of trying to make it impossible, but her conversation with her bhabi indicated that was not the case. (The best revenge plot for Ayra was to let Zaid divorce Mubashira, take him out of his family and business, make him work for Asif's rival and let Asif rot - girl needs lessons on 'How to take revenge?' or writer needs a wake up call!!). Anyway, in all honesty, she may not have known that a 'second marriage' would have been served to her - her fault was agreeing to that. She went into it willingly - so, she is also responsible!
Now coming to the man of this triangle - from the beginning of their marriage, Zaid threatened Mubashira with a divorce and he also told Ayra that he will divorce MJ - but he left me wondering what he is waiting for, especially after Aamir's death (that 40 days condition)? And when he came to his father (the scene where Asif disowned him) and said he wanted to marry Ayra - his intention was clear, he wanted a second marriage, which again plants the doubt whether he really wanted to divorce MJ. Yes, he blackmailed Mubashira with what he knew mattered to her the most, though I was not sure that he actually believed that MJ will bend and compromise that much, but his hand in creating the 'polygamy' mess is definite!
