I have partially addressed this in my post on the episode.

Sai still is stuck on that day when she left CN. She hasn't moved. She is angry with him, but deep down has feelings for him.

Whether Sai would have returned if V had come looking for her is debatable. She herself may not be able to answer that question accurately. After the accident she would have been really vulnerable with so much intense emotions and mourning for her lost baby. So it could have gone either ways. Maybe she would have given in considering she was going to be a mother and the way V would have apologised and repented. It is subjective.

But now, it is not just what happened between them when she left, but the fact that V did not even bother to look for her after the accident. Which proved her fears right that he didn’t really care for her, and whatever he said and she derived from it was correct. So her anger is manifold.

Now it is all the more difficult for him to prove her otherwise.

Have I missed something? Why does Sai not think about how she lost Vinayak in the accident? How did Vinayak end up with Chavans?

It has not been explicitly shown, what happened post the accident.

But today there was a dialogue by her, which states that she thinks either the baby is lost or died. She is not aware that Chavans managed to find him.(if he is not adopted). If she knew about him, she would have definitely returned to CN.
