Nandu sees them fighting and she thinks she add fuel by pretending to get sick. Ram gets worried and requests Pihu to bring Nandu’s medicine box. Pihu melts seeing Ram’s worry for Nandu and assures him Nandu will get fine and gives him a hug 🤗

I think the next revelation will be about Nandini's medicines. As per the precap, Pihu accidentally took Nandini's medicines. Most likely those are those fake medicines in the white bottle.
