Talaq = divorce. I don't see any difference. But in Pakistani dramas talaq always equals to 3 talaqs, and means that the couple can't return to each other.. although in Islamic rules, Talaq has to be once.. then the reconcile, another talaq then reconcile, but after the 3rd talaq they can't reconcile unless she marries another man and gets talaq from him.

Was she pregnant? Any way divorce can happen any time, in surah Al Talaq , it is explained iddat for pregnant's lasts untill they deliver.😊 No iddat without talaq. So it happens.

Yes. Khula is a way for the woman to end the marriage while divorce as in talaq is a complicated concept where husband has to talaq his wife three times and then if he wants her back she has to do halala.
