I think while trying to pull open the drawer with a lot of force causes him to bump his back against the wall where the exposed nail is (the head of nail...)... technically with enough momentum from the pulling the drawer he could've landed on the wall & "stabbed" himself. ... Kind of wish they came up with another reason he needed to be in the hospital though. Everything about this nail scene seems silly.
