If image is in png format then it isn't getting uploaded. Happened with me too

Do not create another quiz. The old one was fine. Just need to set the thumbnail.

Done. It is now Active.

One more thing. If the Correct answer is single you need to choose Single Choice.

Multiple Choice option is to be selected when you have 1 or more correct answers.
