I have recently read girl on the train and women in the window. Both the narrators were unreliable in their narration and definitely manipulative liars. So this, kind of makes me wonder?
I am not a health professional, but just based on what I have gathered :
Mental health is an umbrella term that covers a wide range of ailments. (Just like any "physical health" ailments) . Most types can have varying "degrees" of severity and whether you can "manage" on your own or be "high functioning" depends on the severity, person-to-person and the kind of help you have access to (be it through the financial means or emotional support of your family)
For example, if people have severe social/general anxiety disorder, it can manifest "physically" through panic attacks. In that case, you might need more intensive counseling sessions .
In case of depression, for example, based on the degree of severity, the "cure" can range from counseling to pills and even ECT.
Wrt to your post, I don't know how or if the books attributed the unpleasant aspects of their personalities to some mental health issue.

The narrator lied to themselves, thus they were unreliable. They mixed alcohol with their medicine and didn't follow through their doctor advise. They loved their imaginary life and showed significant resistance towards real people who wanted to be their for them.
