I think Tabu really died in the accident and he got her eyes. So yeah he lied to Radhika again

But was he one-eyed like that bunny? Or could he see with both eyes? That bunny head on his stick was intriguing, a clever touch.

There was no accident. And neither did Tabu kill Swami. They simply take her organs and Ayushmaan gets his eyesight back.
