Kunal is lost and confused and is frankly speaking a hopeless case at this stage of the show. He doesnt know what he wants, when he was with Mauli he was crazzy for Nandani and now when he is with Nandani his thoughts are somewhere else.

I guess he wants what he really can't have, first it was Nandani and now Mauli.. forbidden fruit it seems. it's always tempting to yearn for what you can't have.

anyways, he should have been clear in his mind before playing both of the women.

being a doctor his behaviour is lacking maturity, responsibility and reflection on his behaviour. Not able to understand that the wrong step can cause serious damage.

but I still dont know if he regrets being with Nandani or if he only feels bad for Mauli. Because he is thinking about Mauli yet promising Nandani that he will return and that they will eat once he is back with her.
