What may have transpired is - Sakshi came to know about PooSh affair and advised Pooja to mend her ways and stick to Adi. Pooja agreed to her mom and tried to maintain distance from Yash and it was during this period that the baby was conceived or maybe the Baby was conceived earlier. Yash continues to pursue Pooja and she may have mentioned to him that she is pregnant and cannot continue... then they go and see a doctor and are not yet decided about the baby... but later Pooja decides to keep the Baby and hence did not sign the divorce paper. Perhaps she was going to meet Yash one last time... and this happened

I do believe that Yash and Pooja never had a physical relationship else CVs will not make Zoya call their love "paak... that would be too big an inconsistency
