Karthik was already like 7 maybe when Suvarna Manish got married so the brother can not be Manish's son, unless Manish and Suvarna had an affair before he got married to Karthik Kirti mother, so there are minimal chances of a half sibling. If it is a kid from her first marriage it is going to be repeating the same thing like you said.
Since they haven't shown Suvarna hugging a picture and only a doll, I was thinking maybe Suvarna was pregnant and had a miscarriage or something.
