We know that Om had a drug? related problem in his past, and also a strong relationship with his mum. So whilst Om used to take care of his mother, Shivaaye took care of Rudra.
He feigns innocence to make his brother's happy. I think he knows that both his brothers can do anything for him, and even though Om and Shivaaye get along, they were more directly involved and you can see they don't see eye to eye on most of the things that happened, although they love each other a lot.
So maybe the first few times Shivaaye used to joke around Rudra, Rudra didn't know as he was young but as he grew older he understood who it was. But Shivaaye always was smiling and laughing when he did that and got it wrong, so Rudra pretended he didn't know what was going on, to see his brother happy.

As a kid, Rudra probably didn't understand the fight but as he grew, but I think he still had panic attacks.That's why Om didn't look like he was shocked at the fact that Rudra was having a panic attack. He was more concerned about getting him out of the panic attack. So I'm guessing thats why Shivaaye took him Rudra to his room. That's also why Shivaaye and Om always look so concerned when Rudra comes to them.
Shivaaye and Om found their solace in their younger brother, so Rudra accepted his responsibility to stay and act as their kid, to solve their unhappiness, whereas Shivaaye and Om did whatever they could not to make Rudra get into tension.
