Possible reasons :

1) Sanams deep concern for Aahil could prevent her and than Dilshad might decide to take down Billi on her own !

2) CV's probably want to stretch Sanam finding out about her past storyline for as long as they can😕

In the video Karanvir posted on instagram, there were two gurneys being pushed in the hospital. The second one was Aahil so I think the first one was Dilshad. I think that she'll either be unconscious for a while and will therefore not be able to tell Sanam, or when she finds out that Sanam and Aahil are married, she'll try to expose Tanveer on her own as Aahil might not believe her if she flat out tells him that his precious "Ammi" is a sociopath and a murderer.
