I think Raman genuinely meant what he said when he said the three magic words "I Love You" ..It's just that he's too chicken to accept it under normal circumstances and say it out loud...
We only heard the doctor advise Raman to treat Ishita like a kid if she's behaving like one ..and then she said something which we as audience didn't get to hear ...Maybe she said something like say something that will make her happy and distract her away from thinking about her hand ..obviously the doctor didn't tell Raman to say "I love you!" ..so it was Raman who came up with the idea to speak his mind out and see how it plays out ...
When Ishita's hand got better ..he was to embarrassed to further dwell on the subject and changed the topic back and got back to his taunting self to escape from reality! ..He was a little extra taunting by asking her to move quickly to the couch etc...only to make up for his mushy behavior earlier which he knows to be genuine! ..
Also he went to check on Anirudh ..just to see the guy that assists Ishu and not to really get him to help her out :-)
Just my imagination maybe ..but I like it better this way ...this Raman makes more sense to me than one who cannot love Ishu after everything she's done for him!! ..
