Shouldn't she be throwing up and showing already? I mean the way pregnancy is typically depicted in ekta serials.

Only when it suits the story line. If she did that, the cat would be out of the bag, Naren would feel obligated to stay with Ankita, Pari could never be manipulated by Rushali if she knew Ankita was pregnant so for all these reasons it will remain a heavily guarded secret until all is well in the Anken world.
