In Spain children always have two last names, the first is the fathers name and the second the mothers name.

i.e. Roberto Diaz Gonsalvez + Sara Martinez Ruiz get married and have a child called Jorge,

little Jorge's last name would be Jorge Diaz Martinez, with BOTH Diaz and Martinez being the last names.

If little Jorge was to grow up and marry someday his child would be called Jane Diaz Doe. It's always the first of the two names that's being passed on.
And no not everyone has a middle name, the whole middle name thing is predominant in the U.S only come to Europe and you see that most don't have a middle name.
I don't have a middle name either.

So she just goes by her last name as Vohra? Do we know this for sure?

Why is this so important to you? What does it matter what her real last name is. Most Punjabi women are 'Kaur'.
