In Cicero, Indiana, a woman's ex-husband drops off their daughter Katie. After Katie's father leaves, she admits that she saw monsters at his house. Meanwhile, the ex-husband goes home and enters his workshop. The table saw starts up by itself. He turns it off and starts to go, but it starts up again. He goes over to figure out what's wrong with it... and someone comes up behind him and shoves him onto it.

Sam is on the phone discussing demon banishment spells with Bobby. Dean has gotten word about the most recent of several mysterious deaths in Cicero and wants to check it out. He admits he's also interested in checking in on Lisa Braedon, an old girlfriend he left there eight years ago.
The brothers arrive in Cicero and Sam drops Dean off at Lisa's house in a gated community. He discovers she's throwing an eighth birthday party for her son Ben. As Dean talks with Ben, he realizes the boy seems curiously familiar: he likes chicks, hot cars, and food, and dresses and looks a lot like Dean himself. Meanwhile, Lisa is talking to Katie's mother, Diana, who is worried that Katie doesn't seem to be herself. Dean asks Lisa about Ben but she says he isn't Dean's son. She's more concerned about a series of strange accidents that have been happening in the neighborhood.
Sam goes to the local diner and the mysterious blonde woman who helped him and Dean shows up. She says that Sam should check on his dead mother's "friends" and then they'll talk again. After she leaves, Dean calls to tell Sam about the "accidents."
That night, Diana notices her daughter's reflection in the mirror and it briefly twists and distorts before returning to normal.
Sam canvasses the neighborhood and talks to another woman whose husband died in an "accident" when he fell off a ladder. The woman says that her daughter was present when her father died. Sam notices that the woman has a mark on her neck, red marks on the windowsills, and the daughter watching from an upstairs window.
Diana is making breakfast and rubbing at a mark on her neck. When she realizes what she's doing, she checks the mark in the mirror. Katie pounds on the door, demanding to be let in. A realtor stops by and Diana quickly gets rid of her, and then turns to her daughter who is demanding ice cream.
As Dean is checking out the neighborhood, he spots Ben. Ben tells him that a bully stole his video game. Dean tells him that he needs to stand up for himself and tells him where to go for. Ben approaches the bully, knees him in the groin, and takes back his video game. Lisa, who has seen the entire thing, tells Dean that he has no say in Ben's upbringing and should stay away from him.
Diana tells Katie they're going for ice cream. Instead she pulls over by the side of a lake, gets out, and lets the car coast into the water. Katie simply sits there, looking back, as the car goes underwater. However, upon returning home, Diana finds her daughter already there, soaking wet but acting as if nothing happened.
After checking the mark against research books, Sam figures that the kids in the neighborhood are Changelings. Those particular demons kidnap children, shapeshift and take their place, and then feed on the cerebral fluid of their parents through the neck. The Changelings are only vulnerable to fire. According to the legends, Changelings keep their kidnap victims alive and imprisoned underground.
Realizing Ben is at risk, Dean drives to Lisa's house but Ben and Lisa both tell him to stay away. Figuring a Changeling has already taken Ben's place, Dean checks the windows and red dirt on the windowsills. Remembering a construction site he saw earlier with red dirt, Dean takes Sam there and they check the house. In the basement they find the abducted children, locked up in cages. Sam goes upstairs and finds the realtor, who wonders what's going on and why he's intruding. However, Sam spots her demonic reflection in a mirror and realizes she's a Changeling as well: the primary one that spawned the rest. Sam manages to drive her off using fire.
At Lisa's house, "Ben" asks his mother to play with him. She notices his reflection reflected in a glass tabletop and realizes that he's not really Ben.
Dean frees Ben first and the two of them rescue the other children, as well as the realtor who is being kept prisoner while a Changeling takes her place. However, the Changeling realtor comes downstairs after them.
The Changeling Ben goes after Lisa, who runs outside. However, the other Changeling children are out there. As she goes back inside, Changeling Katie attacks her mother.
The Changeling realtor attacks the Winchesters, who tell Ben to get the real kids out. Dean manages to drive the Changeling back long enough for Sam to kill it using fire. Once the progenitor is destroyed, the others are killed as well.
Later, Dean takes Ben to Lisa. He tries to explain what he does. In return, Lisa tells him that she had blood tests done and determined that Dean isn't Ben's father. Dean says he's proud of Ben and admits he's been thinking about leaving her behind. However, when Lisa asks him to stay, Dean says that he has a job to do and leaves.
Researching Mary Winchester's past, Sam learns that all of her close friends have died as well. He finds the blonde woman and demands to know what's going on. The woman admits she's a demon but insists that she is there to help Sam. She isn't sure what's happening, but insists that if they can find out, they can save Dean.
