I am way too late to add few cents in this thread however here is my opinion...
I love Karan Nandini...was happy they married at the end but their story was made dirty...
Starting with Karan sacrificing his love...he took the decision alone which ruined not only his life but had a bad impact on nandini's life which she called out as well...she went through marital rape...however moving on from there...karan accepted nandini's kid...he said he will give kid his name...when Nandini mentioned she cant love the kid as kid will remind her of Ansh...karan mentioned that even if nandini doesn't love her own kid he will give enough love to kid that it doesn't matter if she loved the kid or not...actually that boosted nandini to actually give birth to bhoomi else she would have aborted the kid...nandini had unwavering support and love of karan
They got married...she gave birth and slipped into coma...he hated the baby coz he thot his love fell in coma bcoz of kid...that is when tanya entered again taking care of bhumi...
Entire virani clan pushed Karan to get married to tanya but he disagreed and then final push came from tulsi...he married tanya
I would say its both karan and tanya fault here...he was an adult in love with someone else...yet just coz his family wished he agreed to marry...he cud have just said no...he even backed out in mandap but again went ahead and married tanya...why but why? he could have just backed out knowing he was doing wrong as he was marrying another woman when he was in love with nandini...And tanya she knew what she was stepping into...she knew karan loved nandini a lot , she saw him backing out from marriage in mandap...even after all this she married him...both consenting adults made a huge mistake
From story POV karan didn't develop feelings for tanya before or during consummating his marriage with her...He actually developed feelings later...
He consummated as tulsi again asked him to give wife rights to tanya...
He could have just said no but again he agreed to his mother's wish and did the deed...but the morning after he actually had regrets of consummation but he covered up the situation to not make tanya feel bad abt it...Tanya noticed all this yet she ignored everything...
Tanya Karan Consummation was not out of love...may be it was for Tanya ...but for Karan he was just fulfilling his responsibility coz tulsi asked him to...there is a difference between love and responsibility...so Karan was morally wrong to consummate his marriage when he was not in love with Tanya...however later he actually started to develop feelings for her...they were happy actually...and nandini came back exactly on their anniversary...Karan just ran back to nandini...in one instance he even says that he will erase all those memories from his life which would hurt nandini implicating he wud just erase tanya's memories... Tanya hears to this conversation...she feels bad coz karan was mentioning abt erasing her from his life and she was abt to share her pregnancy news with him...she becomes dull....karan actually notices all this yet he ignores tanya and goes to nandini...from all this it was obvious that no one cud ever come between him and nandini...nandini was his life and love...Tanya knew from beginning abt his love for nandini yet she married him...it was her mistake and Karan shud not even have married her since his life was nandidni...
Then fast fwd a bit...KN were living in shanti niketan...karan was a bit hesitant to go close to nandini may be coz he was scared how nandini would react knowing he married tanya ...and later tanya leaves shanti niketan...and probably we can assume that KN consummated their marriage (not shown though but nandini was shown to be pregnant so we can consider that whe tanya left it paved way for karan to shed his inhibitions and move closer to nandini again)
But meanwhile Karan was taking bhumi to meet tanya...it was morally wrong..hiding it from nandini and taking kid...and during this time Tanya was pregnant she should have reveled it to karan but she didn't...even though she would have mentioned I dont think Karan would have left Nandini to go back to Tanya...
However Karan taking bhumi to meet tanya or speaking to her hiding it from nandini all looked like two timing...though he had not been living like a husband with Tanya he still had lingering feelings for tanya during that time, he scolded nandini once when she wanted to search a groom for tanya...
It was more like he loved nandini a lot...he was looking and giving nandini her rights of being his wife...but again had lingerings feelings towards tanya...asking her to give him sometime to fix everything but not accepting her as his wife...
He was keeping tanya hanging ...it was too wrong...like he cant give her rights of being wife so tell her straightforward but he was asking time...nandini's condition was also not right to reveal all truth and even if he had revealed everything to nandini I think he was never gonna accept tanya as his wife again because during the time when he used to take Bhumi to meet tanya he had mentioned to her..."Dont expect from me that we can have a kid together...it wud be different before nandini came back from coma but now its impossible" implicating to tanya that she should never expect any kind of husband wife relationship from Karan...he had feelings for her but was not ready to accept her...he asked time to fix everything....like dude was saying I cant accept u as wife but I will fix everything...like seriously? so it was Karan who was at fault to keep tanya hanging...and tanya was dumb not to understand anything/ ignoring though she understood everything....but fast fwd a bit Tanya was not shown for a long time , neither it was shown karan was in touch with tanya...so we shud assume that he completely got over his feelings for tanya and was happy with nandini...
But again fast fwd...nandini went to Jail...Karan again married tanya...this time again Karan time and again said listen I cant give u the love and place nandini has in my heart and life...he even implicated that if he wud marry her it wud be more of friendship kind of thing...he was straight forward...and now it was sole fault of tanya...knowing all this again she married him...if she married she should not have expected anything from Karan...but she expected that Karan would love her again which he never did...he became somewhat cold towards all family members after tulsi's death before second leap still he was treating tanya fine like a fried but after jail term nandini didn't return to him...he became way too cold and held tanya responsible for nandini not returning...he was wrong to hold her responsible but tanya was wrong to expect karan wud happily play family family with her when nandini went to Jail
And I also hold karan and tanya both responsible for hiding truth from bhumi abt nandini...nandini was bhumi's mother she shud have known truth...either one of them cud have said truth...but no one did...
However all said and done....this is my understanding - Nandini was Karan's true love and she was his life...he was shattered after she went into coma...he gave in to family's pressure and married Tanya...life is too long to spend alone...may be he gradually started to accept that nandini wud never return...doctors had mentioned same...but he was adamant to keep her alive with life support...so may be in those 2-3 yrs he accepted the reality that his love wud never come back...and meanwhile he saw Tanya being selfless mother to bhumi...and since he got that push from tulsi considering nandini wudn't come back he moved on with Tanya...but nandini actually came back...and Karan went back to her...
The reason I think Karan didn't love tanya again after Nandini went to jail was he knew this time aroud Nandini was alive and she will surely come back to him after serving her jail term ...so he didn't accept Tanya as his wife and when she didn't come back and tanya was expecting him to play happy husband and wife he grew cold towards her...
And Tanya - The first time she married him , she expecting him to love her was somewhat not wrong because everyone believed Nandini wouldn't come back...it was mentioned by doctors that nandini would never come back and its of no use to keep her on life support but karan was adamant and didn't want to let nandini go...he kept her in life support...so everyone believed Nandini wouldn't come back...her expectations were not that wrong..she thought probably she could live a happy life with Karan as nandini would not come back...but second time when she married karan she very well knew abt the situation...she knew nandini would come back to her rightful place after serving jail sentence...she knew she was actually a care taker/nanny this time...it was too eveident yet she jumped into this relationship ....and when she knew the realityl she should not have expected Karan to love her...she did and her expectations were not met and she turned negative when nandini came back and karan went back to nandini and same time around bhumi and manthan too started leaning towards nandini...
Tanya had built a delusional castle around her which broke as soon as she met with reality....thatz when she turned negative
Tanya turning negative stems from her own delusions...she wanted Karan to love her which didn't happen..she knew everything yet she stepped in loveless marriage and when her expectations were not met she turned negative...

and other 2 kids will be below 10 as per logic but this is ekta verse so I dont know how old KN kids are
