The pain he felt on his arm wasn't even half of the pain he felt in his heart. They showed that Ranbir's lover betraying him wasn't as deep as the physical pain he faced. The creatives put so much depth into his character that even our heart's were touched by this sad moment.

This is actually Kanya's dead sister's fault becuz something big must of happened in Kanya's life between her and Natasha(dead sister) that caused the split personality disorder.
