I think it is time for Prem to know the truth abt Heer.
They should show some scene in which he sees her with the wedding card written by her father along with Prem's father when they were children.
Of late Heer is betraying herself that she loves Manmeet.
So Prem is like disheartened and then it should come out like an explosion that Heer loves him from when she is born. They should show her confession to hers elf which should be watched by Prem from behind with tears.
