Prerna's scraches..........marks........?

Why can't Prerna keep a trained nurse to treat for Prem.Rather than keeping a country untrained person. By this way KZK goes to 19th century.......?
Even Sneha was not treated properly in her drug case......Easily Anu Prerna could go to clinics with her....but they went to fix Prerm & Mukti......
but she forgot some essencial duty?
