Yes, maybe Ryan's here imagination and she is inflicting bruises on her self...for every mistakes she makes.

And maybe Arnav is trying to make her believe that Ryan is dead by throwing his corpse in the ocean. Maybe she will stop visualizing him now.

I even would stress on the theory that her step mother and Ryan are her imaginations, she and Arnav are husband and wife and she is going through trauma where she is hurting herself her father died or she faced an abortion. She was already a schizophrenic but her condition worsened and Arnav got her on a treatment to help her through it.
