I also give a thought about it once thinking about all these spots on his face what are they really? then found no answer let it on his left cheek there's a mark designs in the form of a little rectangle I think what was the need.
If ever someone has such spots why this person will not try to find a solution in this advanced today science world instead of thinking that he's inferoir to others, and in the show Madhav is also well educated.. Rahul should also helped...

WEIRD is only his hair cut...

Those are marks of chicken pox. When we were kids, we all had chicken pox and had some pox marks on face too but our mothers applied Neem-Haldi ka lep on those spots and they disappeared. Perhaps Madhav had chicken pox as a kid, and Bharti didnt apply that medicine of neem haldi thats why the spots remained and only got darker and bigger with age.
BTW did Madhu have those spots when they showed him as a kid? I dont remember...must see the school play scenes again!

Or it might be some sort of hereditary skin problem as some have suggested here. Im sure with dermatologists help those spots can clear up with medication or creams. But Bharti doesnt care for external appearance and she would never mention skin treatment to Madhav and he also doesnt care for such things.

I think here also the cvs show the makeover after aradhya fully accepts madhav beast became handsome prince when princess kissed him and in the same way aradhya will change this beast into handsome man (beauty & dermatologist treatment) that will boost his confidence from the makers of the same ph we saw a heroine tuk the hero into foreign country and he won the competition so we can expect the same magic here also

dark skinned people also look good .. so to make him look ugly they introduced those spots otherwise Madhav would have ended up looking dashing hero...
yeah true that they did not explain those spots as no one gets it by birth (i think so) .If it was due to chicken pox then also it would have got clear by now as he was having those spots in childhood also (1st episode)..

hope this mystery is soon resolved!!
