Because the reality is that actors will get work after being violent or whatever other scandals and they will eventually be praised for how they've turned their lives around or whatever. A doctor cannot be on national television beating and cursing or whatnot and then turn around and say I've learned my lesson.
Moreover if you look at the Physician's Oath, it is based around the idea that doctors must always remain humble, must act in a way that respects their teachers and those who came before them, and must dedicate life to the service of society, without having an ego or a superiority complex. When Umar comes onto a national/international platform as Dr. Umar Riaz, the suffix attached to his name is a responsibility and he carries that with him, so it's good to remind him of that.
Lastly, it would be beyond hypocritical of Salman Khan to say to an actor that angry or violent behavior will ruin their careers, he's living proof that isn't the case.
