Yes… first they went extreme with Akshu almost being lost. She had stopped breathing twice, Abhi had imagined her burning on a pyre and then they give us this. Are these writers for real?
Well yeah those were symbolism plus making Abhi (and us) aware that she is very vulnerable and has some trauma in her background - Medical people would have that knowledge/awareness that panic attacks are associated with extreme trauma/emotional response to a certain trigger. For Akshu that is abandonment by her loved ones. Her first attack came at the thought of leaving and the second one most likely due to leaving Abhi (and this world!) Yes I know she was unconscious, creative liberty at it's best.😆
I'm not versed in Hindu mythology but doesn't the pyre burning represent the end of the mortal body? the symbolism here, to my mind is that, the soul goes to it's home but the body Abhi is so attached to is forever gone from him. Don't forget, the preceding episodes showed his madness/passion for Akshu. So here she is near death and he's heartbroken that she might go away from him to a place he could not follow willingly (like he has been doing all this time!). Then we see the result of his passion when he lays vulnerable in front of his god. We see his vulnerability and she is it.
TV writers have to show these visually and some show it well and others not so much. Response is instant ie trp so they can quickly change tack or switch things around hence sometimes some episodes are silly and others good. If they had the chance to stick to their original theme and premise we would see more authentic stories on TV.
