I remember what Raman was going through when he thought he might lose Ruhi to Shagun during the earlier episodes. He shed tears for her non-stop and was obsessive about her. I think he also takes it for granted that Ruhi will always be his and will be looked after well by Ishita.
With Adi, he is insecure and his heart yearns to have his son near him. Not just because Shagun took him away, but also because he knows that Adi is not getting the same love as Ruhi that he is entitled to. Adding to that Bala's observations didn't help Raman feel any better.

Shagun always made Raman feel inadequate about money and power, but she can no longer do that. So on a personal front, she is trying to do that in terms of Adi and Ishita. She is also playing with his mind in terms of breaking Ishita's trust, because I think that's her main goal here - using Adi to hurt his and Ishita's relationship. Shagun knows that no self-respecting woman would tolerate the kind of betrayal that she is coercing Raman into, so when Ishita finds out things should fall apart.

Before this, she was always convinced that Ishita came second, she is just an 'aaya' for Ruhi, and that any affection Raman showed for Ishita in public was just for Shagun. The hospital scene really got to her in a different way, because Raman was in pain and he instinctively reached out for the person he felt secure with. That was not for show and Shagun knew it right away. It was the eye-opener that she needed to get out of her delusional world that Raman still cares about her, and his cordial approach with Shagun was the icing on the cake.

Shagun is an attention seeker. It may seem as if she cares about Adi, but I still believe she took only him because he would be a weapon to keep Raman in check. Ruhi would have cramped her style and she shipped him off to boarding school in order to live her life. It was Ashok who brought him back. I don't think she would have done it of her own accord.

Now that Ashok is not doing as well, she is losing her interest in him slowly. I think deep down she does regret leaving Raman, not because she cares about him or their children, but because the grass isn't greener on the other side. Raman became all that she admired in Ashok and more in terms of professional growth, but his personal life seems to be going great. Shagun has to deal with loneliness, no family or real friends, and a partner who she is increasingly sure won't marry her. Where does that leave her? A loser. She can't go back to Raman because of her ego, but I'm sure if she set her mind to it, she could use Adi to bargain with him and come back into his life by moving Ishita out of the way. Whether or not she can do that is another story.

In all this tug of war we have 2 innocent children Ruhi and Adi who hate the other parent, one because of Shagun's brainwashing and the other because of seeing Shagun's true nature. What a mess!!
