Becuz Viren is the only doctor capable of saving Karan and if Abha refuses to marry him he wont operate Karan and leave him to die. The idea is that since Viren lost his most priced person (Neelu) due to Karan, Karan must lose his most valued person (Abha) to him. Tit for tat u see :) Hope I helped.
