Nani may not be alive but what if she had come back from her coma and had opened her eyes...hmm only to see nanaji hugging Nirmala and in a frustration and haste she pulled off her oxygen mask...when nanaji saw her she was already gasping for breath or probably breathing her last... that's why Nirmala said why not tell the truth... Nanaji thinks if the family comes to know of his true colour he will lose all the love and respect and all will be probably shattered.
