Its just a wild guess dont take it too seriously
when Rk got hurt on his head in ballu's death sequence may be he got some internal head injury before going picnic he gets his report thats why now he dont want madhu to suffer becoz of his health thats why faking it .

i wish they didn't show this is all revenge i want grey Rk but not like that which make no sense n it will ruin Rk's char
