Well, Smriti was not exactly 'paagal'
It was more like fear.....

Smriti had health issues from the very beginning of the show!
Things started to get serious when she found out about her parents death. It effected her soo badly that it caused her pregnancy to be done early (and the child was born premature)

Smriti starts to lose her memory after giving birth. The doctor gave some pills to Mihir to give to Smriti. The pills were supposed to help. Mihir trusted his mother so much that he asks her to take care of giving her medicine at the right times always.

Mihir's mother takes advantage of this and does not give her the pills. This causes Smriti to have serious memory loss.
Not paagal. How she got paagal was all due to the pressure that her mother in law continuously gave her. Since Smriti had memory issues, lots of incidents occured where she forgot her baby (at the mall, kidnapped, etc) Smriti's mother in law takes her baby away from her and Smriti becomes paagal from there.
