Had they moved it before medical aid arrived he could have had a very heavy blood loss, his body could have gone into a shock and his heart might have failed within minutes.
Remember he’s oozing blood from the mouth. So there’s internal injuries, the cupboard was both the cause of the injury and also keeping a high enough pressure on him that the blood flow was controlled. His ribs have taken a significant impact for sure if not been fractured or broken, that’s why the internal bleeding happened almost immediately. He spat out the first mouthful of blood which landed on Tulsi, so it’s his airways expelling the blood from his mouth.
Move that cupboard with the proper medical care that even an ambulance can provide in an emergency, limited as it is, it would help him reach the hospital. Remove it before you have medical help and he could die on the way to the hospital even if he’s alive when he gets into the ambulance.
Our body doesn’t understand what is happening on its own and in the process it starts releasing the potassium and magnesium in our body as the muscle cells break and the rupture leads to their containment ions to be broken down leading to a flow that’s both uncontrollable and too much in volume all at once. It immediately goes to the heart and messes up the rhythm that the heart is normally supposed to have for a person dependent on their age, gender and fitness level. Whatever is regular for you is amplified by ten times if not more. This leads to immediate cardiac failure.
For someone in his 60s as Mihir is, fit as he is, his internal organs are bound to work a little slower than a man in his youth, say 20s like Parth. This means the shock and the impact can kill Mihir likely in 5 minutes too whereas Parth or Dev might still see 15.
What worsens this impact is when the magnesium and potassium reach the brain. Mihir already likely has head trauma, as the cupboard did hit his forehead, the oxygen flow is disrupted by the injury to his chest affecting his lungs and ribs. The brain can die even faster in a way than the heart as the lack of oxygen is causing it to shut down. The heart can be shocked back into action and activity even if it stops if even a basic CPR is given to the patient quickly but there is nothing that can be done about the brain once the oxygen supply is either cut off or depleted to a dangerously low level. You are taking a dead man to the hospital in that case whether you know it or not.
