It was both Vanraj and Kavya's decision to have Ema and both are guilty but Anupamaa mostly blamed Kavya. Because she decided to stay as an unpaid maid even after divorce and provide free service Vanraj and his parents.

The hidden impact of her actions was seen through causing turmoil in Kavya's married life and ensuring that Vanraj is never happy with her. But her quest for revenge was not over even after both of them ran away.

In the context of Rahi, it's (name change) a lost cause. Anupama's spell is cast on her. Isse Anuj aur poora Kapadia khandaan, Asa Bhawan nahin bacha, Kotharis kya cheez hn. Aisa na ho, Kotharis par bhi bhaari pad jaaye.

Your interpretation is on point. May be she is taking revenge from both Kavya and Crapadia for uprooting her from her beloved family by turning their beloved daughters into vile and screwed respectively... Both are doomed.
