Coming to emotional aspect, heart when used in such conversation generally symbolize the inner feelings and emotions and hence used as a metaphor for that (that's how this show will take it) In most of the cases these inner feelings and emotions (will call the heart henceforth. Heart won't refer to physical body part) drives the behaviour, the attitude and even decisions.

Imagine sitting in a chariot which is driven by 5 horses and a Saarthi. You are sleeping/inattentive. In that case the saarthi will drive you wherever he wants. He will drive you at his will, if it's nice it will take you correct path else a wrong path
However if you wake up, you take the control, you decide where chariot has to go.. Saarthi now just follows your orders.
So this chariot is your physical body/individuality, the five horses are your sense organs and heart is your saarthi. You are the soul..
Since most of the time you (soul) sleeps in journey hence the decision of your heart becomes final. It takes you to follow the path it wants. Still it's not "you"
Here Yash will get Neil's heart, that will mean that the driver of both Neil and Yash are same. They will behave very similarly. But there would be instances when Yash will be awake and then his behaviour would be his not of Neil's
Similarly while Neil was alive, he would have at times behaved differently from his heart.
So to answer your question, no Neil wouldn't be with her, but the main driver which made Neil to be Neil will be with her
