according to cricinfo it was actually shanto who realized the timeout thing and then he told shakib, toh shakib ne tab appeal kiya

yeh shanto ko jitna bhola samajta tha utna hai nahi

Really difficult for England to qualify for champions trophy now since Bangladesh and Srilanka both have much better NRR than England, difficult for England to beat them in one Nederland match...

Unless England wins by some 200 margin (assuming NZ Sri match washout and each getting one point) they can't push Bangladesh down..
