I will say if this was Akshara 1.0 she would just use the calm approach and because Abhi can be stubborn this would have taken weeks to get him to come back to the hospital. The fact she did try to call out his extremes shows now the ability to have some communication. Sometimes you cannot sugarcoat everything. Essentially (I know some people won't like this reference) she is talking like Abhi. How he would call someone out if they were being unreasonable or stubborn about something.
But there is a double standard going on if Abhi ever said anything like this to anyone he is immediately labeled toxic.

.....She wasn't talking like Abhi. He is blunt and doesn't sugarcoat things but he never berates a person to the level that Ak has been doing since a long time now....least of all a person who is anyways struggling with pain and guilt.
Let's just rewind back to a few instances when Ak was being stubborn/unreasonable and Abhi was the one trying to push her out of that zone. Back in Kasauli when she jumped out of his car and refused to ride with him to go give her law exam....he used reverse psychology and reminded her that she needs to get her law degree so she can fight with him and he won't go easy on her so she needs to fight and win over him. More recently, after Nav's death, when Ak was having panic attacks and berating herself as a failure, he never once said ki haan you are a failure only, you are sabki problem, you are selfish blah blah.....instead he simply asked her to face the fact that she also needs to process her pain and she shouldn't hide in a shell like Abhir is. There is a STARK contrast between Abhi and Ak's approaches when they are trying to push someone to come out of a mental rut.
I just caught up on this epi and this entire incident was again shown in a such a twisted manner....lol Ak seemed way more focused on whether the girl that Neelamma sent to get her surgery done needs Abhi as a doctor or someone else could do
....only after hearing those people still wanting Abhi as the surgeon, she ran to lash out at Abhi
. And when he asked her why she's so worried about that girl, she twisted the whole topic to claim she is more worried about HIM
....there was nothing leading up to that scene to indicate this is all coming from a place of her worrying for Abhi's future as a doctor, his wajood blah blah....lol but when she saw that he wasn't willing to do that girl's surgery, she suddenly changed her whole approach to claiming he needs to do the surgery more than the girl needing it 
Even a simple track of Ak supposedly wanting to support Abhi is also shown in such a twisted manner by the writers
.....this is why I can only laugh at these scenes after a while....Ak has to be shown saying twisted things for the heck of it by the writers to always dilute whatever she may be doing "sincerely" also....lol
And yes, the irony is that Abhi is labeled toxic for not sugarcoating things but Ak is called an angel for kicking someone who's already down
....she is hailed as a savior for using twisted words/beratement as a "technique" to "help"
.....her motivations are always left looking shady by the writers
