Tanmi, that was in the beginning days of their marriage, would say 3 months after they got married in the show. The accident was the turning point of their relationship, which led to their love story for the next 8 months and confession happened just before their marriage turned 1 year. For the next 1 year after that it was all handling family problems which led to the postponement of their SR until their 2nd anniversary. The education track happened 6 months after that, nearly 2 1/2 years after the show started. Here they are showing it sooner than the OG and all the other versions.

Now coming to answer your question about an accident, they already showed an equivalent to it in the form of kidnapping after the first marriage.

But this time if Rishita or Janu feel that PS is doing much better than PAGSO and both become insecure, they could show Janu plotting against Shiva and Raavi. We cannot truly say if it's going to be an accident, just have to wait and watch.
