To be frank, here Akshara was still slightly conscious when Abhi operated here. She remembers hearing him and the feeling of him there.

At the fire, Abhi was knocked out fully with smoke inhalation which can cause delusions.

There is a bigger chance of Abhi remembering their drunk confessions than the fire confessions.
