Not just that scene. The entire episode was raghav telling something, doctor instructing otherwise to pals and pals looking at angry rr and then ultimately agreeing to whatever doc said.

Pals knew rr will be pissed that's why before and after going against rr, she kept looking at him. Kya karen when miya biwi not on the same page

I guess his sanki baap told him about Pallavi. So that is why he. held her hand and maybe he added one plus one two and deduced he remember Pallavi's name because he is her husband and his already antagonistic relation with Raghav and his ego made him hold Pallavi's hand. If he is really Mandar then it is normal you can forget your memory but not your basic character trait!!
