@bold, from my experience, a lot of times, people make mistakes because they don't really think they are doing any harm. A mentor of mine told me life is like playing a game where you don't know the outcomes. You could say something as simple as "You look beautiful" to someone it it can be a positive outcome where they become happy or it could be a negative outcome where the person becomes triggered by those words.
However, at the time of an apology, you know a mistake has been made. It's much harder to put together a broken vase than drop a vase. Even after you put together a vase, it might not be the same, and I think that's how apologies are as well. Sometimes, even after apologies, you don't return to the original relationship, and I think that's the most scary/nervous part of it all.
This is just my two cents. There are others who knowingly make mistakes for the thrill, but those are different types of people altogether.
