My heart wants him to have gotten that hurt by saving/defending Raavi.

I think it's either Janardhan or Rishita. If at home with Rishita, he may have gotten in spat with someone and if someone accidentally pushed him, he could have hit his head on the well.

It could also be Janardhan if they were out of the house.
