Also, how would Alexander feel knowing that she made his enemy her brother? Would she do it out of genuine affection for Puru or just to spite her husband?

Some people have said on the forum that this would be reason that Puru spares Alexander, but it's also possible that things happen the other way around and Alexander spares Puru's life because of Roxanne.
