Do people who can't afford toilets even go and watch Hindi films in theatre? I have never understood how films like Peepli Live and Toilet Ek Prem Katha are going to solve social problems.

Don't know if they'll watch this movie but yes they go to movies, a lot in some cases

Going in the open is also a cultural issue, as they show in the movie itself (trailer)

First you've to change people's mindset

It is not about cannot afford. It is about not wanting to change habits. You can see in the trailer itself. Akki's dad(the character) was against having toilet, not because he is poor or cannot afford. It is because simply he does not want to have toilet in his compound where also growns tulsi. It is not only about not being able to afford but also negligence towards sanitation in India.
