I don't think Dev's loyalty to her will be an issue at all. Since he loved her, he has been firm in his heart that he loved her.
I guess he would be very sad that she has fertility issues. It is much much worse psychologically for the woman involved because it will shake her to the core.

Without a doubt, he will stand by Sona if Ishwari has issues. But will he realise the magnitude of a blow it will be for Sona and help her through it firmly? that I dunno. He is so clueless when it comes to understanding her feelings.

Also the fact that till date, not once did he tell Mamiji to stop calling her Bangalan.
