Yes. Gaurav is really Prem. The whole story is that Gaurav died during the bomb blast and Prem didn't. While Prem was at the hospital, he was informed that he has a 'hole' in his heart, which is a lie. So, then Prem comes back to help the Juneja's as Gaurav Sharma, so then when it is time for him to leave, then there will no one to stop him.
