Pia's new Gynaec

Did Pia not have a lady Gynaec before-the one who went on and on about how to take care of the mother. From where did this new doctor spring up? And if he knew who the real father was (which is what Pia would want us to believe) why is he not surprised that Pia and Puskar have come together rather than Pia and Jai?
