He could have divorced Nandini before marrying Tanya. Nandini was still his wife. It looks even worse that he was with someone while his wife was sick. I always had problem with husbands leaving wives when they are sick for any reason, and this character does the same. I can’t believe Tulsi also agrees. In childhood, I thought Karan and Tanya were only married. Didn’t know Nandini was still his wife or him and Tanya had a child until the discussions I came across here.
Also someone who watched Kyunki told me Karan also wrote in diary that he fell in love with Tanya which Nandini read at the time

I don’t remember the diary scene exactly but I think it’s vaguely the scene where Nandini finds out Karan was married to Tanya. He doesn’t write he’s in love with her but that he feels conflicted and uneasy about the whole situation. After Nandini wakes up she’s not supposed to be stressed and told the truth about Karan Tanya as the shock can affect her health very badly. Karan is very happy to see her awake again, but he doesn’t try to make any advances towards her. When they got married she was heavily pregnant and he obviously didn’t try to consummate their marriage given her condition. When she does wake up, he’s living with Tanya as a husband should. They’re happy together and he’s consummated his marriage with her. He’s been intimate with her for months or about a year, she’s pregnant when Nandini wakes up and is about to inform him that he’s gonna be a dad, by surprising him on their anniversary with the news as a gift when Nandini wakes up and Karan gets a call from the hospital. He doesn’t know until months later Tanya is pregnant with his child.
Karan didn’t feel comfortable being intimate with Nandini as he felt that he was wronging two women as it is. But with one he had crossed this line and now, Nandini wants him to touch her but he can’t do it. When he tries to stay away from Nandini, she is hurt and feels like Karan is probably disgusted with her given what Ansh did to her. So, he’s essentially caught in a situation where if he consummates his marriage with Nandini, he cheats on Tanya and also Nandini in a way. If he doesn’t do it, he is being faithful to one woman at least but Nandini ends up getting thoughts into her head that again can impact her health as she was slipping into depression thinking Karan was rejecting her and being disgusted by the notion of even being intimate with her because she wasn’t a virgin.
That’s the context of the diary scene iirc. Karan isn’t confessing his undying love for Tanya there. He’s simply pouring his heart out there to give his thoughts some outlet.
