I've seen that scene and watching well written shows like The Sopranos I've learnt to read between the lines as well , see DB said in that scene that he will tkae care of Prem's family , these kinds of stuff are usually said when people are in their deathbed and the doctor clearly said that Prem would get better and his condition is stable ; DB was litterally saying "I will take care of your family after I'm done murdering you ,beta"

My suspicions were confirmed during the hospital track when Yuvika was doing the aarti and got struck with the realisation that she needs to investigate from the hospital....DB was the only one who heard her and alerted Vidur.....also Vidur is ONLY loyal to DB, he said that himself , DB is the only person for whom Vidur would take the fall for hence all the Riya murder stuff and Vidur letting himself be caught happened , why would he do it for someone else?

Also this is just a tv show so if my theory falls flat it's fine by me

Its that hoodie mask spy in last week episodes who alerted Vidur from hospital on a phone call when Vidur was just relaxing at home, that Yuvika is now in SP hospital & she came to know about 'aapka FAKE nurse' so she is trying to check old cctv footage.. then Vidur asked him to disturb cctv footage in hospital, while she was checking.

It's Yuvika MAHAJAN. Her MARRIAGE if Mahajans arrange tho Swastik will go bankrupt upar se DB k Guru Ji ka aalag natak.
