Nobody from the show “as in the characters like Dr Sai or any other extraordinary doctor” said anything remotely related to infertility.

This character has huge comtrubution to the story and his small presenge changed the entire dynamics..

First he harasses sai and virat got transfer to GC leaving patralekha

He kidnaps sai then virat leaves his brothers wedding in the middle..

He kills KJ and SaiRat had to get marry

He killed Samrat and SaiRat lost their first child which led Virat choosing Pakhi over his wife...

He was protecting virat's wife and his kid all these years but misunderstood by Virat that led Virat drowning towards p..

He revealed Savi's truth which changed the entire dynamics between all

He al revealed Vinu's truth Sai which caused sai slipping away from Virat..

Everything is for entertainment unless it's not for the most realistically beautifully flawed male written ckt ever Shri Shri Virat Chavan or the actor playing it or the ship which is a bigger brand than Reebok, nikey, puma etc.
