Its all about Akshu's fears. She made this illusionary family for Abhir. Abhinav represents for her at this point her only support, her shield from the truth, and Abhir's father. If the truth is revealed what happens to Abhinav? It means her world collapses and Abhir loses this father who she has been pushing on him for 6 years. Her lies will come out and she will lose possible trust from her son. She will lose everything. And she is also indebted to Abhinav as well so there is a big mixture of things going on.

But Akshu has dug her own grave with the choices she made and her stubbornness to continue walking down this path. Its like the saying "Oh what a tangled web we weave when we first practice to deceive" Granted she had her reasons due to the hurt and anger over how Abhi reacted. But now with Abhir's sickness she has a chance to tell the truth. But Abhi should also man up and apologize fully for his behavior and admit how he wronged her that day. Not that he should expect anything in return.
