Most people think Dev and Rishita will have another child, a son, and the fourth boy is for Shiva and Raavi. I think they are basing this upon the audition videos on youtube.

We still don't know for sure what exactly Shiva's condition will be but yes, I also think it will be too much to show Raavi also being pregnant and dealing with everything alone.
